Responsibilities
- Coordinate care across primary, dental, behavioral health, and wellness services.
- Perform triage and patient assessments.
- Develop and implement individualized care plans.
- Provide patient education to promote health and wellness.
- Collaborate with an interdisciplinary, faith-inspired team to advance health equity and quality outcomes.
- Document thoroughly in the EHR to support continuous improvement and quality assurance.
Qualifications & Skills
- Registered Nurse (RN) licensure.
- Strong coordination, assessment, and patient education skills.
- Ability to work effectively in a team-oriented, mission-driven environment.
- Commitment to providing whole-person, affordable care for underserved populations.
