About the Role

Covenant Health HomeCare and Hospice, East Tennessee's largest locally owned home health and hospice provider and part of the largest nonprofit health system in the region, is seeking a Clinical Field Staff Supervisor (CFSS). This role ensures the overall coordination of home health and hospice services, facilitating relationships between referral sources, patients, caregivers, and employees to deliver care according to acceptable standards of practice.

Responsibilities

  • Manage Referral Intake to ensure clients receive timely assessment visits per agency policy.
  • Lead Case Conference for HomeCare and IDG for Hospice, addressing recertification and discharge decisions based on conference findings.
  • Review, approve, and attach Case Conference/IDG coordination notes to Episode Detail Reports, entering them within 48 hours of the meeting.
  • Contact physicians to obtain orders for continued or add-on services; review, approve, or decline orders as they appear on the action screen.
  • For hospice, approve level of care orders and ensure appropriate criteria are met; enter and approve all orders for tracking and route to Medical Records for physician signature.
  • Update client medication profiles and schedules via orders; ensure calendar modifications are completed for frequency-based orders and verify existing orders for requested medical supplies.
  • Follow up on unsigned orders from the Order Tracking Report and enter detailed Non-Admit information in HCHB Coordination Notes when no visit is made.
  • Review and process vital sign alert reports and wound score deviations, documenting actions, follow-ups, and physician notifications.
  • Receive lab reports, assess for normality (if not an LVN/LPN), and forward to the PSC for physician faxing.
  • Communicate among departments for Quality care and Process Improvement; take administrative on-call to support the on-call nurse, managing intakes, evaluation packets, and visit scheduling during off hours.
  • Participate in on-call rotation and provide ongoing supervision and evaluation of assigned Homecare staff.
  • Review and follow up on entitlement verification issues and benefit authorizations for non-Medicare patients, documenting actions in Coordination Notes.
  • Review on-call coordination notes reports each morning and follow up on billing claims audit deficiencies within 24 hours.
  • Oversee Notification of Missed Visits submissions to physicians and act as backup/resource for the Patient Services Coordinator in rescheduling visits.
  • Assist with timekeeper approvals and scheduling of CTO time; analyze staffing needs and submit recommendations to Clinical Manager.
  • Assure accuracy, completeness, and timeliness of all documentation; develop action plans for staff with documentation deficiencies and initiate disciplinary process as appropriate.
  • Schedule regular staff meetings in collaboration with PR/QI for education, problem-solving, and communication; identify process improvements and report to Clinical Manager.
  • Initiate disciplinary processes for staff not meeting professional/ethical standards; keep Clinical Manager informed and seek guidance.
  • Actively participate on agency committees, task forces, and teams; serve as manager designee in the Clinical Manager's absence.
  • Participate in hiring new employees, collaborate with PR/QI on performance appraisals for clinical staff, and ensure new employees complete required orientation and training.
  • Participate in community awareness programs as necessary; perform local travel and other related duties as assigned.

Qualifications

  • Current and maintained RN license in the state of Tennessee.
  • Hospice Supervisors must have and maintain Hospice and Palliative Care Certification (may be obtained within one year of employment).
  • Valid Tennessee driver's license Class D and state-mandated minimum insurance coverage; driving record must meet Covenant Health minimum standards.
  • One year of home health or hospice experience required.
  • Three years of home health field staff nurse experience preferred, with broad experience in high-tech equipment and knowledge of Medicare guidelines.
  • Excellent communication, conflict resolution, teambuilding, organization, and delegation skills; ability to confront difficult situations effectively.
  • Preference may be given to individuals with supervisory or management experience.
