The Client Stabilization Specialist works at shelter or temporary lodging sites to stabilize individuals and families experiencing homelessness, conduct HMIS intakes, provide orientation and education, perform face‑to‑face visits and perimeter walks, escalate stabilization issues, coordinate services for complex needs, complete required data entry, and identify housing prioritization needs.
Responsibilities
- Work with individuals and families experiencing homelessness at shelter/temporary lodging sites.
- Ensure individuals/families entering shelter/temporary lodging are stabilized.
- Conduct intakes into HMIS.
- Provide education on rules and responsibilities, resources, and overall orientation while in shelter/temporary lodging.
- Perform face‑to‑face visits, perimeter walks, and escalate issues impacting stabilization.
- Communicate with leadership and Client Care Coordinators on program violations to ensure issues are addressed.
- Report identified needs of individuals with complex substance use, mental health, housing, and medical care needs to leadership for proper service coordination.
- Complete data entry into the established data system (violations, end‑of‑shift notes, room checks).
- Identify the need for completion of a housing prioritization assessment for all individuals.
Requirements
- High school diploma or GED required.
- Minimum of twelve months of recovery from substance use and/or mental health disorders required.
- 1‑3 years of full‑time health care related work experience where prior experience provides adequate exposure to both
