About the role
We are hiring a Client Care Coordinator to serve as the operational hub of our Private Duty home care program. This role is ideal for someone who thrives in a fast-paced environment, genuinely enjoys working with people, and takes pride in delivering a consistently excellent client experience. This position works fully on-site at our Lombard office and collaborates daily with the Private Duty Administrator/Manager, playing a central role in client intake, caregiver scheduling, staffing, call triage, and overall service quality.
Responsibilities
- Schedule and manage daily and weekly caregiver schedules for all active private duty clients.
- Fill open shifts promptly and manage caregiver call-outs and last-minute cancellations.
- Match caregivers to clients based on certifications, skill set, language, and compatibility.
- Maintain accurate scheduling records in the EVV system and produce daily coverage reports.
- Serve as the primary point of contact for private duty clients and their families.
- Conduct intake interviews, gather required documentation, and onboard new clients.
- Perform regular proactive check-in calls to assess satisfaction and emerging care needs.
- Triage inbound calls, resolve complaints, and escalate clinical concerns to the appropriate team.
- Support Partner with HR and the Private Duty Administrator/Manager to identify and address staffing gaps.
- Aid in caregiver interviews, onboarding paperwork, and pre-employment verification.
- Assist with job posting, recruitment outreach, and caregiver retention initiatives.
- Maintain complete, HIPAA-compliant client and caregiver records in the EVV/scheduling system.
- Monitor caregiver credential renewals, in-service training compliance, and annual health requirements.
- Complete incident reports and unusual occurrence documentation per agency policy.
- Provide daily scheduling status reports and weekly coverage summaries to the Private Duty Administrator/Manager.
- Support operational workflows, intake procedures, and process improvement initiatives.
- Participate in rotating after-hours on-call coverage.
Qualifications
- A high school diploma is required; an Associate's or Bachelor's degree in Healthcare Administration, Human Services, or a related field is preferred.
- Minimum 2 years of experience in home care, healthcare coordination, client services, or scheduling.
- Experience with EVV platforms (ClearCare, or similar) is preferred.
- Strong communication, follow-up, and relationship-building skills.
- Organized, detail-oriented, and able to manage multiple priorities simultaneously.
- Calm under pressure with sound judgment in urgent or emotionally sensitive situations.
- Proficient in Microsoft Office Suite; ability to learn care management systems quickly.
- Bilingual is a strong plus.
Compensation & Benefits
- Base salary: $48,000 - $58,000 annually (DOE).
