About the role
The Client Care Coordinator will assist with generating new clients, coordinating client on-boarding, improving client care outcomes, and maintaining high levels of client satisfaction. This role serves as a key liaison between clients, families, caregivers, and internal teams to ensure quality care and seamless service delivery.
Responsibilities
- Complete Life Profile assessments, care plans, first visits, and reassessments in the homes of potential clients or senior communities
- Work within our company CRM and applications to document work completed
- Conduct inquiry calls and follow up to schedule client assessments
- Follow up with and/or complete client visits to advocate for clients and maintain or improve satisfaction
- Collaborate with the Client Service Manager (CSM) to identify high-risk clients and improve care outcomes
- Set and maintain systems to meet KPIs (Key Performance Indicators) related to client conversions and satisfaction
- Communicate effectively with the scheduling team and management regarding new client care needs, preferences, and scheduling requests
- Partner with the Care Team Manager and Clients Services Manager to communicate caregiver hiring needs based on client prospects
- Accurately document all activities with prospects, clients, families, and community/influence centers in the company software system in compliance with state and company requirements
Requirements
- Minimum three (3) years of experience in health care, in-home care, or management
- Must be a self-starter with the ability to work independently and take initiative
- Excellent organizational skills, attention to detail, and the ability to prioritize in a changing environment
- Strong verbal and written communication skills with excellent client and family interaction abilities
- Strong computer and Microsoft Office skills
- Ability to learn and use new computer applications such as our company’s CRM, and documentation software
- Must pass a Washington State background check upon hire and every two (2) years, as required
- Proactive problem-solving and issue-resolution skills
- Must have reliable transportation and the ability to travel to client homes in a large territory
Qualifications
- Minimum qualifications: Bachelor’s degree, Management or supervisory experience, Washington State HCA (Home Care Aide) credential, CNA credential, Experience performing assessments for in-home care, Experience in Sales or Marketing within the Senior Community
- Preferred qualifications: Experience in health care, preferably in-home care, Management or supervisory experience, Bachelor’s degree, Washington State HCA (Home Care Aide) credential, CNA credential, Experience performing assessments for in-home care, Experience in Sales or Marketing within the Senior Community
