Client Care Coordinator

Home Instead is the world's leading provider of personalized in-home care services. We employ compassionate Care Professionals dedicated to enhancing the lives of aging adults and their families. Services include companionship, personal care, Alzheimer's care, and hospice support.

Work Schedule

• Monday through Friday 8:30 am - 4:30 pm
• Hybrid position; travel to clients' homes in Southern Montgomery County: Dresher, Abington, Jenkintown, Horsham, Hatboro, Elkins Park, Oreland, Huntington Valley, and Fort Washington.

What You'll Do

• Develop and maintain positive relationships with clients and their families through quality assurance visits, phone calls, and special events.
• Plan and execute a schedule ensuring each client has at least one quality assurance every 30-60 days.
• Meet with potential clients and family members to discuss their needs and provide solutions through Care Consultations.
• Communicate with professional partners to coordinate various aspects of a client's care.
• Conduct client and Care Professional introductions as scheduled.
• Provide community education on senior care issues and resources.

What You Need

• 3+ years of experience in the Senior Care or Health Care Industry.
• The ability to work independently in a field position, with a self-starting, upbeat, positive, go-getter attitude.
• A vehicle to get to clients in Montgomery County.
• Excellent organizational skills, problem-solving skills, and customer service.
• A team player attitude.
• A warm, friendly, compassionate demeanor with elderly clients.

Pay

$45,000.00 - $65,000.00 per year
