About the role
This role assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
Responsibilities
- Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
- Completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed.

Requirements
- Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
- MSW licensure by the South Carolina Board of Social Work Examiners.
- Certification in case management preferred.

Pay
The pay rate is hourly and falls within the Pay Grade Health-27.
Schedule
The schedule is 8 hours per week.
