Responsibilities

  • Coordinate services for families to ensure they receive appropriate care and support.
  • Ensure all services provided comply with relevant regulations and guidelines.

  • Maintain accurate records and documentation of services provided.
  • Communicate regularly with families to monitor progress and address any issues promptly.

Requirements

  • Bachelor’s degree in Social Work, Public Health, or related field.
  • At least 2 years of experience in case management or social work.
  • Strong interpersonal and communication skills.
  • Ability to work independently and manage multiple tasks simultaneously.
  • Proficiency in Microsoft Office Suite.

Qualifications

  • Valid driver’s license and reliable transportation.
  • Experience working with children and families in a healthcare setting.
  • Knowledge of local community resources and agencies.
