The role of Children's BHH Care Coordinator/Case Manager involves providing comprehensive support to families in need, ensuring their access to necessary resources and services.
Responsibilities
- Assess family needs and develop personalized care plans
- Coordinate services from various community partners
- Maintain regular contact with families to monitor progress
- Provide emotional support and guidance to families
- Ensure compliance with all relevant regulations and policies
Requirements
- Bachelor’s degree in Social Work, Public Health, or related field
- At least 2 years of experience in social service or case management
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office Suite
Qualifications
- Valid driver’s license and reliable transportation
- Experience with data entry and record keeping
- Knowledge of local community resources
