Certified Nursing Assistant - Hospice/Home Health

About the role

Provide compassionate, hands-on care to hospice patients in their homes, ensuring comfort, dignity, and quality of life. Collaborate with RNs and the interdisciplinary hospice team to support patient needs while following individualized care plans and maintaining precise documentation.

Responsibilities

• Assist with activities of daily living including bathing, grooming, mobility, toileting, and oral care
• Observe and report any changes in patient condition accurately and promptly
• Offer emotional support to patients and their families to promote well-being
• Follow individualized care plans and maintain precise documentation of care provided
• Collaborate with RNs and the interdisciplinary hospice team to support patient needs
