Hospice Certified Nursing Assistant/Home Health Aide

The Certified Nursing Assistant/Home Health Aide (CNA/HHA) works under the direction of a Case Manager/DON and is responsible for providing resident care to assigned residents in accordance with resident assessments, care plans, and as directed by nursing management. The CNA/HHA works with other licensed care staff to coordinate the proper types of care to residents and strictly follows all Company nursing policies and procedures as well as safety guidelines.
Responsibilities
- Provide resident care based on assessments and care plans
- Work collaboratively with nurses, social workers, chaplains, volunteers, and leadership
- Work closely with patients on a one-to-one level while receiving support from a patient care team
- Operate independently on a day-to-day basis
- Drive to patient locations using reliable transportation
Requirements
- Certified Nursing Assistant (CNA) certification in the state(s) of practice
- Home Health Aide (HHA) certification in Oklahoma
- Valid driver’s license, auto insurance, and reliable transportation
- Strong commitment to improving quality of life for patients and partnering with community organizations
- Eagerness to pursue higher-level opportunities such as certifications and further education
