Certified Nursing Assistant - Hospice/Home Health

Responsibilities

• Provide compassionate, hands-on care to hospice patients in their homes
• Assist with bathing, grooming, mobility, toileting, and oral care
• Observe and report changes in patient condition to the interdisciplinary team
• Offer emotional support to patients and their families
• Follow individualized care plans and maintain accurate documentation

• Ability to deliver empathetic patient care
• Strong communication skills for reporting and emotional support
• Attention to detail for documentation and observing patient needs
