Responsibilities
- Makes visits to end-of-life patients on Saturdays and Sundays
- Performs supply delivery and triage visits as needed
- Coordinates proper types of care with other licensed care staff
- Follows Company nursing policies and procedures and safety guidelines
- Works independently and collaborates with various healthcare professionals
- Provides care to patients on a 1-2 basis with support from a patient care team
- Works well within a team environment
Requirements
- Valid driver’s license and auto insurance
- Certified Nursing Assistant (CNA) certification in the state(s) of practice
- Home Health Aide (HHA) certification in the state(s) of practice
