Certified Nursing Assistant (CNA) | Long Term Care

Avera Granite Falls Health Care Center – primarily evening shift with rotating weekends and holidays.

About the role

Accountable for providing direct, personal care and services to residents in the skilled nursing facility, meeting physical, emotional, social, and spiritual needs daily to maintain and improve their highest level of independent function.

Responsibilities

• Assist residents with activities of daily living, including bathing, dressing, oral hygiene, bed making, transfers, repositioning, toileting, feeding, and ambulation.
• Perform routine and non-routine duties as directed by nursing staff, such as serving and collecting food trays, answering call lights, and maintaining a clean environment.
• Measure and document vital signs: blood pressure, pulse, temperature, respiration, and weight.
• Perform or assist residents with specific medical care activities per established policy, including restorative exercises and specimen collection.
• Assess and document completed cares; report status or changes in patient conditions, pain management, and concerns to nursing staff immediately.
• Restock supplies and maintain a clean, safe work environment, including cleaning procedures and compliance with infection control and hazardous communication programs.
• Provide emotional, educational, and spiritual support for families.
• Comply with all aspects of resident rights, including confidentiality and privacy; report allegations of resident abuse or misappropriation of property.
• Follow established policies and procedures related to fire, tornado, evacuation, disaster plans, universal precautions, and infection control.

Requirements

• Active Minnesota CNA certification or enrollment in a MN CNA class ending within 30 days of applying.
• Ability to work specified hours, including visual acuity and effective communication skills.
• Ability to perform essential job functions with or without reasonable accommodations.

Qualifications

• High School diploma or GED equivalent preferred.
• Less than 1 year of related experience or training.

Pay

$19.75 - $26.00 per hour, dependent upon experience. Position may be eligible for a sign-on bonus up to $1,000.

Schedule

• Evening shift: 2 p.m. – 10:30 p.m.
• 48 hours per week.
• Every other weekend and holiday rotation.
