Avera Granite Falls Health Care Center – Regular position, primarily evenings with rotating weekends and holidays.
Pay
The pay range for this position is $19.75 - $26.00 per hour, dependent upon experience. Position may be eligible for a sign-on bonus up to $1,000.
Schedule
- Evening shift: 2 p.m. – 10:30 p.m.
- 64 hours per two-week pay period
- Every other weekend
- Holiday rotation
About the Role
Accountable for providing direct, personal care and services to residents in the skilled nursing facility. You will meet residents’ physical, emotional, social, and spiritual needs daily to maintain and improve their highest level of independent function as part of a compassionate, multidisciplinary team.
Responsibilities
- Assist residents with activities of daily living, including bathing, dressing, oral hygiene, bed making, transfers, repositioning, toileting, feeding, and ambulation.
- Answer call lights, ensure a clean environment, and transport residents upon admission and discharge.
- Perform vital signs (blood pressure, pulse, temperature, respiration) and weight; document concise, factual information on appropriate forms.
- Assist residents with restorative exercises and collect specimens as directed by nursing staff; assess and document completed cares.
- Provide immediate feedback to nursing staff on resident status, pain management, and any changes in condition.
- Restock supplies, maintain a clean and safe work environment, and perform cleaning procedures.
- Provide emotional, educational, and spiritual support for families.
- Comply with all resident rights, including confidentiality and privacy; report allegations of abuse or misappropriation of property.
- Follow established policies and procedures related to fire, tornado, evacuation, disaster plans, universal precautions, infection control, and hazardous/chemical communication.
Requirements
- Active Minnesota CNA certification or enrollment in a Minnesota CNA class that ends within 30 days of applying.
- Ability to work the specified hours, including visual acuity and effective communication skills.
- Ability to perform essential job functions with or without reasonable accommodations.
Preferred Qualifications
- High school diploma or GED equivalent.
- Less than one year of related experience or training.
