Job Responsibilities
- Provide direct personal care and assistance with activities of daily living (ADLs), including toileting, hygiene, dressing, mobility, positioning, and other supportive services as needed while promoting consumer dignity and independence.
- Respond appropriately to consumer requests for assistance, emergencies, falls, illness, or other incidents, following established agency procedures and promptly notifying the supervising LPN when clinical intervention is required.
- Conduct routine overnight safety rounds throughout the residence hall, monitor consumer well-being, and promptly report any medical, behavioral, or safety concerns to the supervising LPN or appropriate agency staff.
- Absorb consumers with nighttime and early morning routines, including preparing consumers for scheduled classes, appointments, transportation, or other Training Center activities.
- Observe consumers for changes in physical condition, emotional well-being, or behavior and accurately communicate observations during shift reports and through required documentation.
- Assist with routine health-related tasks within CNA certification and agency policy, including obtaining vital signs, assisting with mobility, and supporting other non-medication nursing activities as delegated by the supervising LPN.
- Maintain a clean, safe, organized, and secure residential environment by ensuring common areas are orderly, identifying potential hazards, and reporting maintenance or safety concerns.
- Complete accurate and timely documentation of consumer care, overnight activities to include shift QR scans, incidents, and observations in accordance with agency policies and applicable regulations.
- Work collaboratively with nursing staff, residential staff, instructors, and other members of the interdisciplinary team to ensure continuity of consumer care and support successful participation in Training Center programming.
- Support emergency preparedness activities by responding appropriately during medical emergencies, fire drills, severe weather, evacuations, or other emergency situations.
- Maintain confidentiality of consumer information and comply with all HIPAA requirements, agency policies, and applicable state and federal regulations.
- Participate in required staff meetings, training programs, professional development activities, and quality improvement initiatives.
Requirements
- A high school diploma or GED.
- Require certification as a Medical Assistant, Certified Nursing Assistant or Behavioral Health Technician.
- May require basic life support skills including CPR, AED use, and ventilations.
