Center Manager - PACE
PACE, a member of Empath Health, is searching for a compassionate and detail-oriented Center Manager to join our interdisciplinary PACE team in Tampa, FL. The Center Manager will provide comprehensive medical and social services that allow seniors to remain safe and independent in their communities. As part of our leadership team, you will make a direct impact on the lives of older adults and their families while helping shape the future of integrated senior care.
About the Program
The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain independent and safe in their own homes. This role offers relationship-based care with real impact, building lasting connections with participants to support their independence and mobility—providing a meaningful alternative to inpatient rehab or traditional outpatient care. Collaborate with physicians, nurses, social workers, and other clinicians in a true interdisciplinary care model focused on prevention and holistic well-being.
Responsibilities
- Lead and supervise day center staff, CNAs, and other assigned team members.
- Coordinate interdisciplinary care planning, family meetings, and participant support services.
- Manage staff hiring, onboarding, scheduling, evaluations, and performance management.
- Partner with leadership to oversee budgets, compliance, and operational standards.
- Act as liaison between PACE disciplines, departments, external providers, and families.
Requirements
- Minimum Associate’s degree with 5–7 years of management experience in a fast-paced, high-volume healthcare environment.
- 2–3 years of progressive leadership experience required.
- Florida professional license (RN, Social Work, Therapy, etc.) preferred.
- Knowledge of the physical, mental, and social needs of older adults.
- Strong leadership, problem-solving, and team-building skills.
- Successful completion of competency assessment upon hire and annually thereafter.
- Valid Florida driver’s license, proof of insurance, and reliable transportation.
- Experience in quality/compliance or managed care/capitated programs.
- Professional background in Nursing, Social Work, Therapy, Dietetics, Gerontology, or related field.
- Demonstrated ability to manage multidisciplinary teams and drive quality outcomes.
