Summary
The Case Worker/CPST Specialist supports adults with serious mental health concerns to enhance their functioning within their communities. Key responsibilities include coaching clients, improving relationships, and preparing for educational and vocational opportunities.Responsibilities
- Affords assistance in conducting screenings and assessments through person-centered treatment planning.
- Aids clients in identifying strategies or treatment options related to their behavioral health disorders.
- Provides supportive counseling and solution-focused interventions to assist clients in restoring stability and adapting to community living.
- Aids clients and family members in identifying strengths, needs, resources, and developing goals and objectives.
- Aids clients in learning coping strategies to manage targeted symptoms.
- Facilitates individual and group interventions as outlined in the treatment plan.
- Develops and implements treatment plans for assigned clients, including medication management components.
- Supports clients in enhancing independence through various life skills training.
- Communicates with other providers for care coordination and resource access.
- Assists clients in managing chronic medical conditions and establishing good health routines.
- Maintains knowledge of community resources and participates in supervision and team conferences.
- Maintains detailed and accurate records in the health record system.
- Maintains professional expertise through ongoing education and training.
- Maintains required client contact time and adheres to agency productivity targets.
- Fulfills agency policies and standards regarding client rights and confidentiality.
- Performs other duties as assigned.
Minimum Qualifications
- Bachelor’s degree in a human service field such as psychology, social work, education, rehabilitation.
- At least 2 years of experience working in a behavioral health or human service setting.
Necessary Special Requirements
- Knowledge of human development and family systems issues.
- Understanding of mental health and substance use diagnoses.
- Knowledge of community resources for clients and families with behavioral health disorders.
- Advocacy and coaching skills.
- Valid driver’s license and use of a vehicle for job-related travel.
- Automobile liability insurance coverage for vehicle usage.
- Ability to collaborate effectively with teams to coordinate care for clients with intensive behavioral health needs.
