Under the supervision of the Clinical Director of Child & Family Outpatient Services (CAFOS) at Gateway Healthcare, this role provides case management to clients and families in active treatment, triage and support for clients in crisis, outpatient treatment adherence assistance, screening requirement support, and documentation timeline monitoring. Responsibilities
- Participates in program-related meetings including staff meetings, clinical conferences, community meetings, and in-services.
- Completes documentation within LifeChart according to established policies and procedures.
- Attends individual supervision with the Clinical Director of Child & Family Outpatient Services.
- Supports the collection and reporting of data required for measuring quality and meeting licensing requirements.
- Coordinates primary care screening requirements for children’s services, including:
- Ensuring the patient has a primary care physician.
- Collecting and reviewing medical screening information with a nurse if necessary.
- Reviewing required screenings with the family and primary care physician.
- Conducting care coordination with the primary care physician (PCP).
- Communicating all relevant information to the clinician or prescriber.
- Provides families with resources such as outpatient therapy, groups, specific supports, and higher levels of care as needed.
- Answers calls from parents/guardians/clients in crisis when their provider is unavailable.
- Maintains an active list of community referral options for additional services and helps identify services for clients who may be graduating or leaving the area.
- Provides support and referral information for family members of active clients (e.g., parents, siblings).
- Runs Epic reports to identify clients who have not been seen in over 1 month (for therapists) or 3 months (for prescribers) and/or have frequent no-shows to support adherence to treatment plans.
- Offers reminder calls to families requiring extra support to attend appointments.
- Supports the Missed Appointment/No Show policy by:
- Reiterating the importance of regularly attending scheduled appointments.
- Providing logistic support when necessary.
- Setting limits with clients/families.
- Sending reminder letters and working with Risk Management to discharge clients according to policy.
- Coordinates care with community organizations to support families in overcoming barriers to treatment such as homelessness, transportation, and medication access.
- Meets with clients one-on-one to complete screenings, work on goal setting, and strategize how to achieve goals.
- Collaborates with clinicians to support them and their clients.
- Enters referrals and screening information in LifeChart as directed by the clinician.
- Initiates medication prior authorizations on behalf of providers.
- Runs reports for clients with upcoming appointments who have outstanding lab work or screenings and provides advance reminders to families.
Requirements
- A Bachelor’s degree in social work, psychology, or other health and human service degrees, accompanied by at least one year of relevant experience in children’s behavioral health.
- Demonstrated knowledge and skills in human development stages and cultural patterns in each step of the care process.
- Bilingual in Spanish preferred.
- Knowledge of community resources specific to the cultural needs of the community.
- Relevant case management experience required.
- Demonstrated ability to interact professionally both on the phone and with colleagues.
Pay
$19.97–$32.96 per hour
Schedule
- Monday–Friday, 8:00 AM–4:30 PM
- 8 hours per day
- No driving required
