Caseworker - Outpatient

Full-Time; Monday - Friday; Days 8am - 4:30pm.

About the role

The Caseworker works in collaboration with the team and assists in the coordination of care for mental health, behavioral and social needs. Facilitates communication between patients, families, healthcare professionals, community and state agencies, and other interdisciplinary team members. Functions as a member of a professional team which provides evaluation and treatment for psychiatric patients.

Responsibilities

• Assist with the implementation of care plans and coordination of behavioral health and social services.
• Provide psychological assessment, crisis intervention, case management, patient advocacy, care coordination, and community outreach.
• Provide input in treatment decisions; present specific treatment plans for approval.
• Obtain information from patients and other appropriate sources to initiate treatment recommendations; record all information in the medical record.
• Involve patient, family, and others as appropriate in the delivery of care and treatment recommendations.
• Work with patients to determine goals to be achieved during the course of treatment.
• Make arrangements for the safe and expedient transfer of patients as required.
• Act as a resource of information for Division members, patients, referral sources, etc.
• Assist with provider schedule maintenance; set up appointments and arrange for follow-up treatment as indicated.
• Support quality patient care, patient satisfaction, and assist in the accomplishment of related goals.
• Assist with insurance verification and authorization procedures.
• Ensure patients' progress in the acute episode of care through post-discharge as part of an interdisciplinary team, focusing on quality, efficiency, and cost-effectiveness.
• Interact with patients, family members, healthcare professionals, and community and state agencies, serving as a liaison between the hospital and community agencies or facilities for clinical and referral information exchange.
• Identify high-risk patients from a psychosocial/financial perspective, assess the psychological needs of patients and families, and provide information, support, counseling, care management, and referrals to appropriate resources.
• Resolve psychosocial patient care issues and develop and implement complex patient transition/discharge plans.
• Comply with all job-specific obligations and organization policies and procedures.

Requirements

• Bachelor's Degree in Social Sciences (Required).
