We are hiring a Case Manager to work with children and families coordinating and facilitating delivery of services in community locations. This includes designing and developing individualized plans in collaboration with community and provider teams and family members to support children and youth to reach their goals and achieve permanency. There is an opportunity for interested and qualified candidates to also be employed with Service Alternatives as a Youth Community Coordinator. Service Alternatives is looking for a Case Manager for a Part-Time position supporting at-risk youth in an in-home setting.
