Job Summary
The IDD Targeted Case Management Lead provides support to Oakland County youth who receive Applied Behavioral Analysis (ABA) and IDD (intellectual and/or developmental disability) services, including waivers (HAB, SED, CWP, etc).
Essential Functions
- Maintains active caseloads of individuals, ensuring the needs of the community and the sustainability of the team.
- Receives and tracks case management referrals and serves as the primary contact for emergencies.
- Responds immediately and effectively to deescalate situations and reports appropriate occurrences to supervisor, law enforcement, and social services.
- Maintains and monitors individual and team performance, documenting and remedying accordingly.
- Ensures appropriate coverage of intensive supportive interventions and facilitates the development and monitoring of a Wraparound Plan.
- Works with OCHN Autism diagnosticians for recommendations, referrals, and case consultations.
- Aids families in applying for SED Waiver services and assists in providing necessary documentation to MDHHS and PIHP.
- Ensures timely completion of SED Waiver renewal process and enters and maintains current (IPOS) information in the WSA system.
- Collaborates with local DHHS and assists with the identification and recruitment of Waiver providers.
- Establishes crisis plans and responds to client requests for mental health coaching or emergency counseling.
- Completes accurate documentation with the OCHN Electronic Health Record System within 24-hours of service delivery.
- Communicates effectively in written and verbal forms with individuals, families, providers, and community partners to ensure coordinated, person-centered care.
- Leads Leeds weekly case reviews/supervisions with case managers and actively participates in the Behavior Treatment Plan Review Committee (BTPRC).
Job Requirements
- Education: A master's degree or higher in social work, psychology, counseling, or a related human services field.
- Licenses and Certifications: Licensure: L/LP, L/LMSW, L/LPC, L/LMFT (Active in Michigan); Certification(s): QIDP, MichiCANS, CWP, ICCW, All HCBS required trainings for OCHN and/or MDHHS.
- Experience: Minimum 2 years' experience in the examination, evaluation, and treatment of minors and their families. Ideally, minimum of 1 year experience in a supervisory capacity.

- Demonstrated ability to work effectively in a team environment, as well as independently.
- Problem-solving skills to identify shortcomings of care plans and revise them accordingly.
- Familiarity with basic PC software, spreadsheets, electronic record management programs, and other software.
- Organizational skills to plan patient appointments and meetings and to coordinate the activities of medical providers.
- Knowledge of trauma-informed principles and practices.

Additional Information
- Must be available for meetings and events which may occur outside of standard office hours.
- Work requires meeting individuals in various settings such as homes, communities, hospitals, schools, and offices.
- This is a community-based hybrid (onsite/remote) position, requiring both in-office and community-based work.
