About the role

We're excited to add a Case Manager to our team. This role plays a crucial part in our mission to prevent and alleviate family homelessness in the San Francisco Bay Area.

Responsibilities

  • Offer personalized case management to participants, guiding them through intake, assessments, and tailored service plans that meet their unique needs
  • Connect individuals with vital community resources, advocating for housing, employment, and support in areas like mental and physical health, domestic violence recovery, and family reunification
  • Design and facilitate dynamic workshops that empower participants with essential life skills and strategies for crisis intervention
  • Welcome families during intake and orientation, collaborating to create actionable plans that address housing, recovery, and children’s needs
  • Build strong networks with community services to ensure participants receive the support they deserve
  • Work closely with case managers, supervisors, and community partners to provide seamless and effective support for families
  • Prepare and submit insightful reports that highlight the impact of services and track our program's success
  • Actively participate in team meetings, training sessions, and community events to strengthen our mission
  • Ensure compliance with all regulatory requirements and organizational policies, and attend hearings as needed
  • Encourage participant involvement in decision-making and program activities, fostering a sense of ownership
  • Take on additional responsibilities as requested to support our mission

Requirements

  • Ability to work and collaborate with a diverse group of people on projects and make decisions for the benefit of our participants
  • Highly organized, self-motivated, and capable of working independently to meet goals
  • Demonstrated ability to exercise appropriate judgement and uphold policies and procedures
  • Skilled in documentation, reporting, and maintaining confidentiality

Qualifications

  • 1+ years of experience in social work, psychology, human services setting or related field
  • A background in working with nonprofit organizations, social services organizations, or community-based programs
  • Familiarity with Bay Area resources for low-income families
  • Bilingual abilities, especially in Spanish
  • Proficient in Microsoft Office Suite, Salesforce, and other software
  • Ability to travel locally and long distance to meet with vendors and participants

Pay

$28.00/hour

Schedule

40 hours with evenings and/or one weekend day required (10:00am to 6:30pm or 10:30am to 7:00pm)
