Position Summary
Unison Health is seeking a Case Manager to join our School-Based team, providing services to youth and families with complex behavioral health needs. This role is ideal for professionals who value flexible, small caseloads, and the opportunity to make a long-term impact through short-term, intensive support.
Key Responsibilities & Role Highlights
- Small caseload – serve a limited number of youth and families at a time

- Time-limited, intensive family support with long-term positive outcomes
- Deliver intensive home-based case management services in accordance with state mandates and agency policies
- Collaborate with clinical therapists to support individualized family service plans
- Document all services provided and maintain accurate client records in the electronic health record system
- Ensure compliance with confidentiality guidelines and proper handling of release of information
Education & Experience Requirements
- Minimum of a bachelor’s degree in social work, Counseling, Psychology, or a related field
- Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
- Valid driver’s license and reliable transportation required
- Must be insurable under the agency’s commercial liability policy
- Experience working with children and families preferred
- Must demonstrate knowledge in:
- Mental illness and treatment
- Individual and group counseling
- Behavioral health assessments and crisis intervention
