Case Manager - Rural Counties

About the role

Catholic Charities of St. Louis, St. Patrick Center, is seeking a Rural Counties Case Manager, working in the Supportive Services for Veteran Families (SSVF) program. This individual will use best practices to assess the housing needs of veterans, who are unhoused or at risk of becoming unhoused. We cover 9 rural counties. The case manager will utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability by meeting with the veteran families to develop an individual treatment plan, coordinate individualized services for housing and financial stability, and document all support related to activities and services. This position will primarily office out of our Festus or St. Charles, MO location and require a clean driving record since the position requires the case manager to meet clients on a regular basis in several counties. A company vehicle is provided, but a personal vehicle may be required intermittently. Core working hours are Monday through Friday 8:00am-4:30pm. Starting salary is $45K annually.

Responsibilities

• Assess the housing needs of veterans, who are unhoused or at risk of becoming unhoused.
• Utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability.
• Develop an individual treatment plan for each veteran family.
• Coordinate individualized services for housing and financial stability.
• Document all support related to activities and services.
• Meet with veteran families to provide support and guidance.

Requirements

• A clean driving record.
• Experience working with veterans and their families.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficient in Microsoft Office Suite.

Qualifications

• Bachelor’s degree in Social Work, Public Administration, or a related field.
• Minimum of 2 years of relevant experience in social services or related field.
• Knowledge of SSVF program and wrap-around philosophy.
