About the role
The role involves providing comprehensive care coordination services to rural elderly patients, focusing on improving their quality of life through health management and social support.
Responsibilities
- Assess patient needs and develop personalized care plans
- Coordinate with healthcare providers to ensure continuity of care
- Provide emotional support and resources to patients and their families
- Monitor patient progress and adjust care plans as necessary
- Participate in regular team meetings to discuss patient cases
Requirements
- Bachelor’s degree in a related field
- At least 2 years of experience in healthcare or social work
- Valid driver’s license and reliable transportation
- Excellent communication and interpersonal skills
Qualifications
- Knowledge of Medicare and Medicaid regulations
- Experience with electronic health records (EHR)
- Ability to work flexible hours including evenings and weekends
Skills
- Strong organizational and time management skills
- Proficient in Microsoft Office Suite
- Ability to handle sensitive and confidential information
