About the role
The Home Health Physical Therapy Case Manager evaluates, develops a plan of care, and treats patients to maintain or restore them to their maximum functional performance level. Additionally, completes discharge planning and provides supervision for Licensed Physical Therapist Assistants and Certified Nursing Assistants.
Responsibilities
- Implements action plans to achieve or exceed service excellence benchmarks
- Evaluates and develops plan of care according to American Physical Therapy Association (APTA) guidelines
- Administers physical therapy treatments appropriate to diagnosis and to APTA guidelines
- Appropriately documents and synchronizes patient care according to federal, state, and local regulations
- Meets productivity standards
- Uses resources (e.g., time, people, supplies, etc.) cost effectively
- Supervises Licensed Physical Therapy Assistants, Certified Nursing Assistants, and students
- Protects patients, employees, and visitors by promoting infection-control and safety policies and protocols
- Plans, coordinates, manages, and documents the care provided to assigned patients
- Ensures appropriate patient and/or caregiver education with utilization of applicable resources
- Leads, communicates, and collaborates with the interdisciplinary team to plan patient care
- Adheres to National Patient Safety Goals (NPSG)
- Participates in performance improvement initiatives affecting patient outcomes
- Handles conflict resolution in an appropriate, timely, and consistent manner
- Identifies personal contributions to department cascading goals
- Practices within physical therapy licensure guidelines
- Provides one in-service annually to staff
- Engages in own professional development as evidenced by self-assessment and attainment of degrees, certifications, and continuing education
- Contributes to providing a supportive environment that is conducive to learning for all students from all academic programs
- Other duties as assigned
Qualifications
- Required Education: Bachelor's Degree in Physical Therapy
- Preferred Education: Master's Degree in Physical Therapy, or Doctoral Degree in Physical Therapy
- Required Experience: For Bachelor's Degree, five (5) years of experience as a Physical Therapist
- Preferred Experience: Skilled Nursing Facility experience
- Required Certifications and Licensures: Licensed by the Virginia Board of Physical Therapy
Pay
Salary Range: $83,588.00 - $125,382.00
