Case Manager Physical Therapy (Full Time, Days, Lynchburg Home Health)

Responsibilities

• Implements action plans to achieve or exceed service excellence benchmarks
• Evaluates and develops plan of care according to American Physical Therapy Association (APTA) guidelines
• Administers physical therapy treatments appropriate to diagnosis and to APTA guidelines
• Appropriately documents and synchronizes patient care according to federal, state, and local regulations
• Mets productivity standards
• Uses resources (e.g., time, people, supplies, etc.) cost effectively
• Supervises Licensed Physical Therapy Assistants, Certified Nursing Assistants and students
• Protects patients, employees, and visitors by promoting infection-control and safety policies and protocols
• Plans, coordinates, manages, and documents the care provided to assigned patients
• Ensures appropriate patient and/or caregiver education with utilization of applicable resources
• Led, communicates and collaborates with the interdisciplinary team to plan patient care
• Adheres to National Patient Safety Goals (NPSG)
• Participates in performance improvement initiatives affecting patient outcomes
• Handles conflict resolution in an appropriate, timely, and consistent manner
• Identifies personal contributions to department cascading goals
• P Practices within physical therapy licensure guidelines
• Provides one in-service annually to staff
• Engages in own professional development as evidenced by self assessment and attainment of degrees, certifications, and continuing education
• Contributes to providing a supportive environment that is conducive to learning for all students from all academic programs

Qualifications

• Required Education: Bachelors Degree in Physical Therapy
• Preferred Education: Masters Degree in Physical Therapy, or Doctoral Degree in Physical Therapy
• Required Experience: For Bachelors Degree, five (5) years of experience as a Physical Therapist
• Preferred Experience: Skilled Nursing Facility experience
• Required Certifications and Licensures: Licensed by the Virginia Board of Physical Therapy
