Case Manager (Phelps)

Program Description

Phelps House is seeking a compassionate, organized, and motivated Case Manager to join our Social Services team. Case Manager will provide direct support and case management services to senior residents, promote their health and well-being, and help foster a safe, supportive, and thriving community environment.

Purpose of Position

The purpose of the Case Manager position will be to join a team of social workers to provide social services that enhance the clients’ overall quality of life and increase their access to a variety of resources. Our focus is on financial stability, health and wellness, food, and housing security.

Candidates Must Have

Monday through Friday 35 hours per week

Salary Range

$47,000 - $50,000
