About the role

This is a per diem position working on an as needed basis for coverage purposes. Ideal Candidate: Case management experience, SNF experience, MA license required (RN, PT, OT, SLP, RT or LCSW/LICSW).

Responsibilities

  • Completes intake interviews; determines need for medical and psycho-social evaluations.
  • Reviews therapist and physician evaluations, care objectives, and plans; coordinates services being provided.
  • Advocates for needed services and entitlements.
  • Communicates effectively and efficiently internally and externally.
  • Coordinates discharge and after care services with guidance from the clinical team.
  • Aids others and supports the team.
  • Demonstrates the ability to follow through with requests, share critical information, and get back to individuals in a timely manner.
  • Initiates a complete assessment, gathers and verifies relevant baseline data, identifies understanding and expectations of the stay, and identifies any potential issues or barriers to arranging post inpatient care.
  • Proactively works with patient, caregiver, and the interdisciplinary team, including community providers of medical services, as appropriate, to develop a realistic and timely plan for post inpatient care.
  • Identifies and coordinates appropriate resources post discharge for patients requiring homecare, outpatient, or placement in an alternate level of care.
  • Performs ongoing evaluation for appropriateness for the facility’s level of care by applying knowledge of level of care criteria with active collaboration with patients’ insurance payer.
  • Complies with all applicable State and Federal regulations as well as other credentialing bodies, including but not limited to, Joint Commission and CARF, regarding the case management process.

Qualifications

  • Bachelor's Degree in related field of study required.
  • Experience case management, utilization review, or discharge planning experience 2-3 years preferred.
