About the Role
Under the supervision of the Director of Case Management, you will participate as the patient program manager of the outpatient/inpatient team of professionals to meet discharge goals of patients and families, and monitor outcomes and utilization of services. You will use effective communication skills by communicating with payor representatives and providing age-appropriate clinical intervention to meet psycho-social and referral needs of identified patients and families.
Responsibilities

Requirements
- Minimum of two years recent experience in a hospital or rehabilitation facility.
- Minimum of two years recent insurance or case management experience preferred.
- MSW or RN required.
- CCM certification preferred.
Schedule
Coverage during Monday - Friday with some occasional weekend coverage; Day Shift.
