Case Manager - Per Diem

Responsibilities

• Clinical Interventions/Discharge Planning: Assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.
• Care Planning Management: Coordinates health care decisions and treatment plans to improve quality and outcomes.
• Fiscal Management: Promotes cost-effective care within the allotted time frame and ensures payments of hospital-based services meet patient-related utilization management criteria.
• Payer/Referral Management: Implements safe and appropriate discharge plans and manages interactions with payers and referrals.

Requirements

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification preferred.
