Case Manager - OPD

Job Description

Provides case management and support to OPD clients
May be required to Facilitates PC groups
Documents according to regulatory standards
Carries a caseload
Functions as part of PC Team
Interfaces with PC psychiatrist
Maintains collateral contacts with family members and providers in the community on behalf of clients

Requirements

Requires bachelor's degree in a human services field
