Responsibilities
- Advocates for, and assists clients with, gaining access to needed medical, social, educational, or other services to address identified needs and achieve specific goals.
- Provides ongoing assessment and support to engage potential clients and promote links to services during a critical engagement point.
- Provides referrals and information to increase education for clients and families on available resources, increase support networks, and coordinate care among multiple service providers such as Drug Court, Mental Health/Substance Abuse Treatment Team, Department of Community Based Services, Juvenile Justice, schools and health care providers.
- Participates as a member of a team established to respond to population specific areas, i.e., adolescent and adult substance use, seriously mentally ill adults, or youth with severe emotional disabilities.
- Maintains regular direct and indirect contact with clients and others who play a significant, vital role in the client’s life.
Requirements
- Required Education And Experience:
- Bachelor’s degree in psychology, social work or other behavioral science related field
- One year of full-time employment experience working directly with adults / children in a human service setting post degree
- Master’s degree may be substituted for this experience
- Deaf and Hard of Hearing case managers must demonstrate proficiency in American Sign Language and must complete a Targeted Case Management certification program offered and approved by the state within six months of employment
- Valid driver’s license, reliable operating transportation, and proof of current auto insurance
- Preferred Education And Experience:
- Master’s degree
- Knowledge of mental health issues, substance use issues, and community supports
- Knowledge of Person-Centered Recovery based interventions in Case Management services

Skills
- Direct face-to-face services to link clients to resources and programs to address identified needs
- Assist clients in developing a Person-Centered Recovery plan of care
- Provide ongoing monitoring to ensure services are being furnished in accordance with the plan of care
- Provide advocacy services on behalf of clients and their families
- Collaborate, coordinate, and provide resources for individual clients as an extension of the team services
- Serve as a community liaison to link clients to services in the community and assist client in building family, friends, and community relationships
- Serve as liaison between clients and the recovery team
- Inform supervisor, and other members of the team as indicated, of issues/concerns related to individual clients
- Attend community meetings and committees to provide education and resources to people in the community
- Engage community partners and advance relationships in regard to mental health and substance use disorder services and education
- Document to meet medical necessity and joint commission standards
- Maintain accurate, thorough records on all assigned cases
- Present documentation in a timely manner to assist clients in evaluating progress
