Case Manager (Weekends)

About the role

The RN Case Manager provides patient care management across the continuum of care. They are responsible for ongoing assessment of patient needs and coordination of the delivery of patient care through the unit manager, Social Worker, and the multi-disciplinary health care team.

Responsibilities

• Ensure patients meet criteria for admission, continued stay, and discharge.
• Focus on facilitating the development of discharge plans and the appropriate movement of patients from one level of care to another.
• Collaborate with Social Workers to ensure a smooth transition to other levels of care.
• Advocate for patients based on defined needs in collaboration with insurers.
• Oversee the evaluation of patient care and treatment outcomes.
• Collaborate with physicians and other health care team members in the practice of care.
• Utilize trended data to focus on issues identified for improvement in quality improvement initiatives.
• Teach the principles of care and case management to patient/families and members of the multi-disciplinary team.
• Assist the Social Worker with complex medical issues.

Qualifications

• Education: 2-year / Associate Degree (preferred)
• License / Certification: Valid OK RN License
• BLS Certified Case Manager (preferred)
• Experience: 1 - 3 years
