Key Responsibilities
- Conduct detailed interviews to identify the psychological, medical, and social needs of each client
- Develop specific, step-by-step care plans to help clients reach their goals
- Provide individual, group, and family psychoeducation-oriented care using various professional treatment modalities
- Cook up individualized discharge planning for patients with physician
- Communicate legal requirements for case management with physician and nursing personnel
- Document treatment provided to patient in medical record
- Report incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency
- Link patients to housing, food assistance, healthcare, and financial aid programs
- Document all client interactions and maintain accurate, confidential files
Requirements
- Master's degree in social work (MSW), Psychology, Counseling, or a closely related health field required
- Minimum of two (2) years of professional case management or social work experience
- Caregiving with excellent communication and problem-solving skills
- Proficient with electronic health records (EHR) and basic office software
- Experience with therapeutic limit-setting helpful or an understanding of level systems or other behavior management modalities helpful
