Case Manager - Master's level

Key Responsibilities

• Conduct detailed interviews to identify the psychological, medical, and social needs of each client
• Develop specific, step-by-step care plans to help clients reach their goals
• Provide individual, group, and family psychoeducation-oriented care using various professional treatment modalities
• Cook up individualized discharge planning for patients with physician
• Communicate legal requirements for case management with physician and nursing personnel
• Document treatment provided to patient in medical record
• Report incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency
• Link patients to housing, food assistance, healthcare, and financial aid programs
• Document all client interactions and maintain accurate, confidential files

Requirements

• Master's degree in social work (MSW), Psychology, Counseling, or a closely related health field required
• Minimum of two (2) years of professional case management or social work experience
• Caregiving with excellent communication and problem-solving skills
• Proficient with electronic health records (EHR) and basic office software
• Experience with therapeutic limit-setting helpful or an understanding of level systems or other behavior management modalities helpful
