The Licensed Case Manager at Central Maine Medical Center (CMMC) plays a crucial role in coordinating patient care and ensuring optimal outcomes. This position requires a current RN license or healthcare professional licensure as PT, SLP, or OT, with a minimum of 2 years of relevant experience in an inpatient setting.

About the role

As a Case Manager, you will be responsible for assessing, planning, implementing, and evaluating individual patient needs, advocating for patients, and facilitating seamless transitions of care. This role involves working closely with interdisciplinary teams, patients, and families to enhance the patient experience and improve quality outcomes.

Responsibilities

  • Assess patient needs and develop care plans
  • Implement care plans and monitor progress
  • Evaluate patient outcomes and adjust care plans as necessary
  • Facilitate communication between patients, families, and healthcare providers
  • Coordinate discharge planning and ensure safe transitions
  • Support continuity of care across the healthcare continuum
  • Ensure compliance with regulatory and payer requirements
  • Advocate for patient rights and preferences

Requirements

  • Current RN license or healthcare professional licensure as PT, SLP, or OT
  • Minimum of 2 years of social work or case management experience in an inpatient setting
  • Acute/rehabilitation hospital experience preferred
  • Preferred certification in Case Management or Rehabilitation Nursing

Pay
