Job Responsibilities
- Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
- Communicate with the resident’s care providers
- Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
- Care coordination and supervision for residents
- Maintain clear and concise documentation in accordance with facility requirements
Qualifications/Requirements
- Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required
- Previous Compuware experience is a plus
- Excellent phone skills and ability to work with families/other care providers
- Strong organizational skills, passion and compassion for the population
- Able to work cooperatively with multidisciplinary team
