Job Responsibilities

  • Maintain firm adherence to the resident’s “Bill of Rights”, as they relate to the facility
  • Communicate with the resident’s care providers
  • Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
  • Care coordination and supervision for residents
  • Maintain clear and concise documentation in accordance with facility requirements

Qualifications/Requirements

  • Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required
  • Previous Compuware experience is a plus
  • Excellent phone skills and ability to work with families/other care providers
  • Strong organizational skills, passion and compassion for the population
  • Able to work cooperatively with multidisciplinary team
