About the role
The Case Manager LPN coordinates patient care, discharge planning, and utilization review to ensure appropriate, cost-effective care across the continuum. This role collaborates with various healthcare professionals to optimize patient outcomes, reduce length of stay, and ensure compliance with regulatory and payer requirements.
Responsibilities
- Cook up individualized patient care plans from admission to discharge
- Conduct ongoing patient assessments and track progress towards care goals
- Facilitate discharge planning, including post-acute placement, home health, durable medical equipment (DME), and community resources
- Collaborate with physicians, nursing staff, social workers, and ancillary departments to maintain continuity of care
- Inform patients and families about treatment plans, discharge instructions, and available resources
- Participate in interdisciplinary rounds to anticipate and resolve discharge barriers
Requirements
- BSW or other bachelor's degree in a clinical healthcare-related field
- At least one year of experience in case management, discharge planning, or nursing management
- LVN/LPN graduate from an accredited school of nursing with a current license, and two years of acute care experience
- Basic to intermediate computer skills
- Minimum five years of acute care experience preferred
- Case management certification within two years of hiring
- One year of experience in case management, discharge planning, or management preferred
- Current BLS certificate preferred
- Knowledge of Milliman Criteria and InterQual Criteria preferred
Qualifications

Pay
- Per diem / PRN hours
- Hours: 8-hour shifts - 8:00 am to 4:30 pm
