Case Manager LPN

About the role

The Case Manager LPN coordinates patient care, discharge planning, and utilization review to ensure appropriate, cost-effective care across the continuum. This role collaborates with various healthcare professionals to optimize patient outcomes, reduce length of stay, and ensure compliance with regulatory and payer requirements.

Responsibilities

• Cook up individualized patient care plans from admission to discharge
• Conduct ongoing patient assessments and track progress towards care goals
• Facilitate discharge planning, including post-acute placement, home health, durable medical equipment (DME), and community resources
• Collaborate with physicians, nursing staff, social workers, and ancillary departments to maintain continuity of care
• Inform patients and families about treatment plans, discharge instructions, and available resources
• Participate in interdisciplinary rounds to anticipate and resolve discharge barriers

Requirements

• BSW or other bachelor's degree in a clinical healthcare-related field
• At least one year of experience in case management, discharge planning, or nursing management
• LVN/LPN graduate from an accredited school of nursing with a current license, and two years of acute care experience
• Basic to intermediate computer skills
• Minimum five years of acute care experience preferred
• Case management certification within two years of hiring
• One year of experience in case management, discharge planning, or management preferred
• Current BLS certificate preferred
• Knowledge of Milliman Criteria and InterQual Criteria preferred

Qualifications

• Hours: 8-hour shifts - 8:00 am to 4:30 pm
