Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III requires additional education, years of experience, and the ability to proficiently perform job functions and meet position standards.

Responsibilities

  • Provide primary support and case management for individuals with mental illness and co-occurring disorders.
  • Assist clients in living independently within their community.
  • Use company vehicle to drive clients to appointments, groups, and other necessary locations.

Requirements

  • Bachelor’s degree in Social Services or Human Services and 1 year of experience, or High School Graduation/G.E.D. equivalent and 4 years of direct services in Mental Health or community services (an Associate's degree may substitute for 2 years of experience).
  • Valid and current driver’s license.
  • Personal vehicle insurance with your name listed as a driver.
  • Willingness to use company vehicle for client transportation.

Schedule

  • Full-Time | AM Shift: 6:00 AM to 2:30 PM | Days: Tuesday–Saturday
  • Full-Time | AM Shift: 6:00 AM to 2:30 PM | Days: Friday–Tuesday
  • Full-Time | PM Shift: 2:00 PM to 10:30 PM | Days: Sunday–Thursday
  • Full-Time | PM Shift: 2:00 PM to 1:30 PM | Days: Friday–Tuesday

Pay

Expected starting wage range: $22.17 - $27.08. Geographic differentials apply based on the location where the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Shift differentials for hourly staff:

  • 6% for PM Shift
  • 10% for Overnight Shift
  • 5% for Weekend AM Shift
  • 11% for Weekend PM Shift
  • 15% for Weekend Overnight Shift

About the Role

Telecare’s Mobile Crisis Response Team (MCRT) is a 24/7 program serving individuals of all ages experiencing a mental health crisis. This innovative program collaborates with County Access and Crisis Line (ACL) and law enforcement to provide crisis triage, risk assessment, in-person intervention, and case management directly in the field in San Joaquin County.
