Case Manager III -Dual Case Management (Dakota County)

Physical presence in the Dakota County is required.

Main Job Duties

• Manage and/or provide consultation on the most complex cases ensuring consistent practices and outcomes.
• Partner with the program leadership on cultivating and maintaining relationships with potential and current referral sources and community partners.
• Provide advanced knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits.
• Affiliate with program management on training case managers on policies, procedures and best practices, taking an active role in onboarding new case managers.
• Perform assessments that lead to individual plan development that result in the client receiving access to necessary mental health and related services.
• Facilitate the development and implementation of a goal plan that addresses the needs of the clients, and is based on their development/goal plan and is culturally appropriate.
• Provide referrals to resources necessary to implement the agreed upon goal plan.
