About the role
Pinnacle Services is looking for friendly, self-starting professionals to join our Dakota County Case Management team. The Case Management department is experiencing rapid growth, and we collaborate with lead agencies across the six-county metro area to serve individuals primarily on CADI, DD, BI, and/or AC/EW waivers.
Responsibilities
- Work with persons served on CADI, DD, BI and/or AC/EW waivers who may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or are aging.
- Assess areas where assistance is needed and offer and coordinate resources.
- Monitor service delivery and work with providers to ensure individual needs are met.
- Develop and maintain service plans.
- Attend regular in-person meetings with persons served and their interdisciplinary teams.
- Maintain accurate case notes and billable units.
- Stay current with case management systems, software, and DHS and County resources.
Requirements
- Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field, or
- Be a graduate from an accredited four-year college with a major in any field and one year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency, or
- Have one year of experience in education or treatment of persons served with developmental disabilities or related conditions, or
- Have at least one course focused on developmental disabilities (official transcript required to verify).
- Valid driver’s license and acceptable driving record as determined by Pinnacle Services policy.
- Vehicle covered with current insurance.
- Successfully clear local and/or state background checks.
Preferred Qualifications
- Possess an LSW or higher form of BOSW licensure.
- For DD Waiver roles: Bilingual in Spanish.

Pay
- Non-LSW wage: $49,000/year salaried for 40 hrs/week
- LSW wage: $51,600/year salaried for 40 hrs/week
