Case Management Assistant III

About the role

The Assistant Care Manager 3 collaborates with the Physician, licensed Psychotherapist, consulting Psychiatrist, and other care team members to ensure appropriate levels of care and coordination for outpatient services. This role reports to the Healthy Young Minds Clinical Manager.

Responsibilities

• Collaborate with the interdisciplinary team to manage outpatient services
• Meet with patients and families to gather and report patient functioning data and clinical measurements
• Utilize the patient care registry and communicate between team members
• Directly contact assigned patients and family members for direct patient care
• Provide crisis intervention and de-escalation support as needed

Benefits
