Case Manager II - Waiver Case Management (Ramsey County)
About the role
People Incorporated is seeking Case Manager II's to join our Waiver Case Management team in Ramsey County. This role works with clients diagnosed with chronic mental illness or other ongoing health conditions, connecting them with services by developing person-centered Support Plans and authorizing services to help them live as independently as possible within their communities. Waiver Case Management involves more behind-the-scenes service coordination than traditional case management. Case Managers see clients twice annually and develop long-term relationships through dynamic service plans. Staff operate autonomously, creating their own hybrid schedules after completing initial training. We value work-life balance and maintain manageable caseload sizes averaging 30 individuals.
Responsibilities
- Attend bi-annual meetings with clients to monitor services
- Develop and maintain a collaborative relationship with clients, serving as a role model by demonstrating positive, proactive, and respectful communications
- Assess client needs and ensure continuity of care
- Maintain regular contact with collaborative partners
- Make referrals and authorize services based on client needs and personal outcomes
- Maintain records and document all services provided
- Excel at documenting all interventions, interactions, and relevant information to the Support Plan consistent with professional and organizational standards
Requirements
- Must be able to have on-site presence in the Twin Cities, Minnesota
Pay
$24.25 - $25.75/hour based on qualifications
Schedule
Full-Time, Monday–Friday, business hours (hybrid schedule after initial training)
