About the role
Accord is seeking a mission-driven Waiver Case Management (WCM) to join a team of Case Managers in making a difference in the lives of people living with disabilities. The WCM will evaluate, create, and support the health and human service needs of people that Accord serves. This is a hybrid role with offices located in Midway St. Paul, MN. The WCM will be expected to travel within the community to meet with supported persons. Staff will work on-site during training and orientation.
Responsibilities
- Develop a community support plan and goals for each person on an assigned caseload
- Meet with each person regularly to evaluate progress and needed changes
- Develop person-center service agreements, with the person, to authorize services to meet the person's assessed needs and goals
- Document changes and adjustments
- Authorize services to meet the person's assessed needs and goals
- Comply with MN Health Care Programs in delivery of waivered services (such as CADI, BI, EW, DD)
- Work with assessors to complete MnChoices assessments
Requirements
- Bachelor's Degree in Social Work, Human Services, Psychology, or related field. If degree is in social work, current MN licensure is required. Bachelor's degree in non-Human Services field must possess one year of related experience.
- Proficiency in Microsoft, Google, and care-related platforms
- Ability to pass mandated background checks
- Current Driver's license and clean driving record with reliable, insured vehicle

- Electronic Health Record experience

- Strong analytical and problem-solving skills with attention to detail
- Excellent written and verbal communication skills including the ability to adapt coaching approaches to varied learning styles
- Customer service mindset
- Ability to work independently and as a part of a team in sharing information and completing work, collaborating with diverse stakeholders
- Strong documentation skills with ability to stay current on high-volume paperwork
Benefits
- Compensation: $47,800-$52,000, nonexempt, depending on qualifications

Work Environment
- 40 hours per week, Hybrid, some travel to in-area locations (limited overnight travel), vehicle required
