About the role
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Responsibilities
- Provide case management and primary support for individuals with mental illness and co-occurring disorders.
- Assist clients in living independently within their community.
- Use a personal vehicle to drive clients to appointments, groups, and other necessary locations (weekly mileage reimbursement at the IRS rate).
Requirements
- High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or
- Associates degree and one (1) year of experience; or
- Bachelor’s degree in Social Services.
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
Pay
Expected starting wage range is $23.00 - $23.24. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
Schedule
- Full-Time | AM Shift: 8:30 AM - 5:00 PM, Monday - Friday
- Shift Differentials: 6% for PM Shift, 10% for NOC Shift
- Weekend Shift Differentials: 5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend NOC Shift

The program is designed to meet the needs of Kaiser members who are high users of inpatient psychiatric services or who are at high risk of acute psychiatric hospitalizations. Services are provided in the community.
