What You Will Do To Change Lives
To change lives, you will serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders. Your responsibilities include:
- Assisting clients to live as independently as possible in their chosen community.
- Providing support and guidance to help clients achieve their goals.
- Collaborating with clients, families, and other service providers to develop and implement individualized care plans.
- Maintaining accurate and up-to-date records of client progress and needs.
- Participating in regular team meetings to discuss client progress and plan next steps.
What You Bring To The Table (Must Have)
- A high school diploma or GED, and two (2) years of direct services in Mental Health or community services; or an Associates degree and one (1) year of experience; or a Bachelor’s degree in Social Services.
- A valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
- A willingness to use your personal vehicle to attend meetings, etc.
