Position Responsibilities
- Conduct initial client assessments
- Complete monthly in-home client assessments
- Develop and implement individualized service plans
- Provide a full range of social services and ongoing case management support
- Travel to clients’ homes throughout NYC using mass transit
- Maintain accurate and timely case documentation
- Collaborate with clients, families, community resources, and program staff
- Connect clients with appropriate services and resources
Qualifications
- Bachelor’s degree in Social Work, Human Services, Sociology, or a related field
- Bilingual Spanish - Required
- Comfortable traveling throughout NYC using public transportation
- Strong communication, assessment, and organizational skills
- Ability to work independently and effectively manage a caseload
- Compassionate, dependable, and community-focused
Interview Process
- Qualified candidates will participate in a Microsoft Teams interview
Pay
$24-$26 per hour
