What You Will Do To Change Lives
To change lives, under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders. The role assists these individuals to live as independently as possible in their chosen community.
What You Bring To The Table (Must Have)
- High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.

Qualifications

Pay
- Expected wage range is $21.65 - $22.52
- The full wage range goes up to $26.36

- Starting pay is commensurate with relevant experience above the minimum requirements.
Schedule
- Full Time; AM 8:00 am - 4:30 pm OR 8:30 am - 5:00 pm; Monday - Friday
