About the role

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Responsibilities

• Provide primary support and case management for individuals with mental illness and co-occurring disorders.
• Assist clients in living as independently as possible within their community.
• Use personal or company vehicle to drive clients to appointments, groups, and meetings.

Requirements

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services.
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments, groups, and meetings.
• Willingness to use the company vehicle to drive clients to appointments and groups.

Pay

Expected starting wage is $23.00 per hour. Telecare applies geographic differentials to its pay ranges based on the location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Schedule

• Full-Time | AM Shift: 8:00 AM - 4:30 PM
• Days: Monday - Friday
• Shift differentials for hourly staff: 6% for PM Shift, 10% for NOC Shift
• Weekend shift differentials for hourly staff: 5% for Weekend AM Shift, 11% for Weekend PM Shift, 15% for Weekend NOC Shift

About the program

The Justice-Enriched Treatment Team (JETT) program serves adults ages 18+ with serious mental illness who have a history of lack of participation in a mental health program that has resulted in two or more hospitalizations or incarcerations within the last 36 months, or have attempted to cause harm in the last 48 months.
